The Pavilion at Bradford Regional Medical Center was permanently closed Saturday.
The previously announced closure plan, accepted by the Pennsylvania Department of Health in back in September, anticipated closing the long-term care facility on or before Dec. 1.
Kaleida Health officials confirmed that all 45 residents who were living at The Pavilion when the closure was announced have all been relocated to other care facilities, with over 95% of them remaining in McKean County.
